Organization Overview:
Our client is launching an innovative Wound Care Division, expanding our continuum of care for geriatric and long-term care residents. This new program presents an exciting opportunity to help shape a specialized care model within a well-established and respected healthcare system.
The Wound Care Nurse Practitioner Division will operate as a dedicated business entity, closely collaborating with existing programs, including Wellspring, Fast Response, Home Health, Hospice, and Palliative Care.
Position Summary:
We are seeking a Certified Nurse Practitioner (CNP) with a specialty in wound care to provide expert clinical management in post-acute and long-term care settings. The ideal candidate is passionate about geriatrics, has hands-on wound care experience, and holds at least one wound care certification.This is a mobile role that requires travel between assigned facilities within a regional territory. You will assess, treat, and monitor wound healing while working in partnership with facility staff to ensure the highest standard of patient care.
Essential Responsibilities:
- Deliver advanced wound care services at designated skilled nursing and rehabilitation facilities
- Assess, diagnose, and treat chronic and acute wounds, including pressure injuries, surgical wounds, and diabetic ulcers
- Conduct regular patient rounds, follow-ups, and chart reviews
- Collaborate with attending physicians, nursing staff, and interdisciplinary teams to ensure optimal outcomes
- Prescribe appropriate treatments and medications in accordance with licensure and prescriptive authority
- Educate staff, patients, and families on wound prevention, care plans, and discharge instructions
- Maintain accurate and timely documentation to meet all regulatory and compliance standards
- Stay current with the latest wound care practices, tools, and technologies
- Adhere to Ohio Revised Code and applicable state scope-of-practice laws
- Maintain all required certifications and credentials, including DEA registration, if applicable
- Travel within the assigned territory (no overnight travel required)
Requirements:
Education & Licensure:- Active RN and CNP license in the state of Ohio
- Certificate to Prescribe (CTP or CTP-E) and DEA registration (or willingness to obtain)
Experience & Certification:
- Minimum of 1 year of hands-on wound care experience
- Must hold at least one of the following certifications:
- WOCN (Wound, Ostomy, and Continence Nurse)
- WCC (Wound Care Certified)
- CWS (Certified Wound Specialist)
- CWOCN (Certified Wound Ostomy Continence Nurse)
