Salary: $60,000 - $75,000 annually
Schedule: Monday – Friday, 8:00 AM – 4:30 PM, with 1 weekend commitment per month
Job Description
We are seeking a dedicated and experienced Registered Nurse (RN) to join our home health care team on the East Side of Cleveland. The ideal candidate will provide skilled nursing care to patients in their homes, ensuring high-quality care and maintaining productivity standards. This position requires travel within the designated service area, extending as far as Cleveland Central, but no further east than Mentor.
Key Responsibilities
- Deliver skilled nursing care to patients in a home health setting
- Assess patient needs, develop care plans, and document progress
- Maintain a productivity standard of 6 points per day using a point-based system
- Work collaboratively with the healthcare team to ensure optimal patient outcomes
- Commit to one weekend shift per month and one winter holiday/one summer holiday annually
Qualifications
- Registered Nurse (RN) with an active license in Ohio
- Minimum of 2 years post-graduate nursing experience
- Prior experience in skilled home health care (HHC) required
- Proficiency with HomeCare HomeBase (EMR) is a plus
- Willingness to travel as far as Cleveland Central within the East Side service area
- Reliable transportation and valid driver’s license
Compensation & Benefits
- Salary: $60,000 - $75,000 per year
- First 90 Days: Paid per visit rates until productivity standards are met
- Mileage Reimbursement: $0.42 per mile
- Health Benefits: Begin the first day of the following month after hire
- Paid Time Off (PTO): 10 days in the first year
- Training: 2-3 weeks of comprehensive training provided
Schedule
- Monday – Friday, 8:00 AM – 4:30 PM
- One weekend commitment per month
- One winter holiday and one summer holiday required
Additional Requirements
- Must maintain a daily productivity goal of 6 points
- Full-time availability only; no part-time applicants will be considered
